About

Irinotecan is an antineoplastic agent classified as a topoisomerase I inhibitor. It exerts its cytotoxic effects by binding to the topoisomerase I-DNA complex, preventing the religation of DNA single-strand breaks, which ultimately leads to DNA damage and apoptosis in rapidly dividing cancer cells. This mechanism is crucial in disrupting the replication and transcription processes essential for tumor growth. Irinotecan is primarily indicated for the treatment of metastatic colorectal cancer, often used in combination with other chemotherapeutic agents.

The drug is administered intravenously and is a prodrug, converted in the liver by carboxylesterases to its active metabolite, SN-38, which is significantly more potent. Its efficacy in advanced malignancies underscores its role in modern oncology, particularly in patients who have progressed on or are intolerant to fluorouracil-based therapies. Careful monitoring for adverse effects, especially myelosuppression and diarrhea, is essential during treatment.
