About

Selumetinib, the active ingredient in Koselugo, is an oral mitogen-activated protein kinase (MEK) inhibitor indicated for the treatment of pediatric patients aged 2 years and older with neurofibromatosis type 1 (NF1) who have symptomatic, inoperable plexiform neurofibromas (PNs). It works by targeting the MEK pathway, which is often overactive in NF1-related tumors, thereby inhibiting cell proliferation and tumor growth. This targeted therapy represents a significant advancement in managing the complex manifestations of NF1, particularly for patients with challenging PNs that cause significant morbidity.

The drug's mechanism involves blocking MEK1 and MEK2, key enzymes in the RAS/MAPK signaling pathway. Dysregulation of this pathway is central to the pathogenesis of NF1. By inhibiting MEK, selumetinib can reduce tumor volume, alleviate pain, and improve functional outcomes in affected children, offering a non-surgical option for managing these often debilitating tumors.

Uses

  • Treatment of symptomatic, inoperable plexiform neurofibromas in NF1.
  • Management of tumor growth associated with neurofibromatosis type 1.
  • Reduction of pain and improvement of functional outcomes in pediatric NF1 patients.
  • Targeted therapy for specific genetic mutations in the RAS/MAPK pathway.
