About

Nebivolol, often available as NEBICARD SM TAB., is a highly selective beta-1 adrenergic receptor blocker with additional vasodilatory properties mediated by nitric oxide release. It is primarily indicated for the treatment of essential hypertension and stable mild to moderate chronic heart failure in elderly patients. Unlike some other beta-blockers, nebivolol's unique mechanism of action, involving both beta-1 blockade and nitric oxide-mediated vasodilation, contributes to its antihypertensive effects and favorable hemodynamic profile. It reduces heart rate, myocardial contractility, and peripheral vascular resistance, leading to a decrease in blood pressure and cardiac workload.

Its high selectivity for beta-1 receptors at therapeutic doses minimizes the risk of bronchospasm and other beta-2 mediated side effects, making it a suitable option for many patients, including those with certain respiratory conditions.

Uses

  • Treatment of essential hypertension (high blood pressure).
  • Management of stable mild to moderate chronic heart failure.
  • Reduction of cardiovascular morbidity and mortality.
  • Improvement of cardiac function and symptoms.

Directions For Use

Take one tablet orally once daily, with or without food, preferably at the same time each day. Do not stop abruptly.

Benefits

  • Effective in lowering blood pressure.
  • Improves outcomes in chronic heart failure.
  • Highly selective beta-1 blocker.
  • Additional vasodilatory effects via nitric oxide.
  • Generally well-tolerated with fewer respiratory side effects.
  • Once-daily dosing for convenience.

Side Effects

  • Headache
  • Dizziness
  • Fatigue
  • Nausea
  • Diarrhea
  • Constipation
  • Bradycardia (slow heart rate)
  • Hypotension (low blood pressure)
  • Peripheral edema
  • Insomnia
  • Depression
  • Erectile dysfunction

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can further lower blood pressure and increase the risk of dizziness or fainting.
  • Pregnancy - Not recommended during pregnancy due to potential fetal harm (e.g., growth retardation, bradycardia). Consult a doctor.
  • Breastfeeding - Avoid during breastfeeding as nebivolol is excreted in breast milk and may cause adverse effects in the infant.
  • Liver - Use with caution in patients with hepatic impairment; dosage adjustment may be necessary in severe cases.
  • Kidney - Dosage adjustment is required in patients with moderate to severe renal impairment; consult a doctor.
  • Lung - While highly selective, use with caution in patients with severe asthma or COPD; monitor for bronchospasm.
