About
Sildenafil is a phosphodiesterase-5 (PDE5) inhibitor primarily used for the treatment of erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). In ED, it works by increasing blood flow to the penis during sexual stimulation. It achieves this by inhibiting PDE5, which is responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. Elevated cGMP levels lead to smooth muscle relaxation and increased blood inflow, facilitating an erection.
For PAH, sildenafil relaxes the smooth muscle cells in the pulmonary arteries, leading to vasodilation and a reduction in pulmonary arterial pressure. This improves exercise capacity and delays clinical worsening in patients with PAH. The onset of action for ED is typically within 30-60 minutes, and its effects can last for several hours.
