About

Sunitinib is a multi-targeted receptor tyrosine kinase (RTK) inhibitor that blocks several key receptors involved in tumor growth, angiogenesis, and metastatic progression. It inhibits vascular endothelial growth factor receptors (VEGFR1, VEGFR2, VEGFR3), platelet-derived growth factor receptors (PDGFRα, PDGFRβ), KIT, FLT3, and RET. By simultaneously targeting these pathways, sunitinib effectively reduces tumor vascularization, inhibits tumor cell proliferation, and promotes apoptosis.

Sunitinib is administered orally and undergoes extensive hepatic metabolism, primarily by CYP3A4, to its active metabolite. Its broad spectrum of RTK inhibition makes it a valuable treatment option for several advanced cancers. Due to its potential for significant adverse effects, including cardiovascular toxicity and hand-foot syndrome, careful patient monitoring and dose management are essential.

Uses

  • Treatment of advanced renal cell carcinoma (RCC).
  • Management of gastrointestinal stromal tumor (GIST) after imatinib failure.
  • Treatment of progressive, well-differentiated pancreatic neuroendocrine tumors (pNET).
  • Used in specific cases of advanced solid tumors.

Directions For Use

Take orally once daily, with or without food, as directed by your physician. Follow the prescribed dosing schedule (e.g., 4 weeks on, 2 weeks off).

Benefits

  • Effective against multiple advanced cancers.
  • Inhibits tumor growth and angiogenesis.
  • Improves progression-free survival in RCC and GIST.
  • Offers a targeted therapy option for specific tumors.
  • Available as a convenient oral capsule.
  • Can reduce tumor size and slow disease progression.

Side Effects

  • Fatigue
  • Diarrhea
  • Nausea and vomiting
  • Stomatitis
  • Hand-foot syndrome (palmar-plantar erythrodysesthesia)
  • Hypertension
  • Skin discoloration
  • Thyroid dysfunction
  • Myelosuppression (neutropenia, thrombocytopenia)
  • Cardiac dysfunction (left ventricular ejection fraction decrease)
  • Liver enzyme elevations
  • Taste alteration

Safety Measures

  • Alcohol - Avoid or limit alcohol, as it may exacerbate liver toxicity and other gastrointestinal side effects.
  • Pregnancy - Contraindicated in pregnancy due to potential for fetal harm. Use effective contraception.
  • Breastfeeding - Not recommended during breastfeeding due to potential excretion into breast milk and harm to the infant.
  • Liver - Use with caution in patients with hepatic impairment; dose adjustments may be necessary due to extensive liver metabolism.
  • Kidney - Generally safe in mild to moderate renal impairment. No specific dose adjustment usually required for severe impairment, but monitor closely.
  • Lung - Monitor for pulmonary adverse events, including interstitial lung disease, though less common; discontinue if severe.
