About

Telmisartan/Metoprolol is a combination medication used for the treatment of essential hypertension, particularly when monotherapy is insufficient to achieve target blood pressure. Telmisartan, an angiotensin II receptor blocker (ARB), works by selectively blocking the AT1 receptor, thereby inhibiting the vasoconstrictive and aldosterone-secreting effects of angiotensin II. This leads to vasodilation and a reduction in blood pressure. Metoprolol, a beta-1 selective adrenergic blocker, reduces heart rate, myocardial contractility, and renin release from the kidneys, consequently lowering cardiac output and blood pressure. The complementary actions of these two agents provide a comprehensive approach to blood pressure control by targeting different physiological pathways. This combination offers enhanced antihypertensive efficacy and improved cardiovascular outcomes for patients with uncontrolled hypertension or those with co-existing conditions like angina.

Uses

  • Treatment of essential hypertension.
  • Management of hypertension with co-existing angina.
  • Reduction of cardiovascular risk in hypertensive patients.
  • Blood pressure control in patients unresponsive to monotherapy.

  • Provides sustained blood pressure control with once-daily dosing.
  • Reduces heart rate and myocardial oxygen demand.
  • Offers enhanced efficacy compared to single-agent therapy.

Safety Measures

  • Alcohol - Alcohol can enhance the blood pressure-lowering effect and increase the risk of dizziness or drowsiness. Avoid excessive consumption.
  • Pregnancy - Contraindicated during the second and third trimesters of pregnancy due to fetal toxicity from the ARB component. Consult a doctor immediately if pregnant.
  • Breastfeeding - Not recommended during breastfeeding as both components can pass into breast milk and may pose risks to the infant. A doctor should assess the risk-benefit.
  • Liver - Use with caution in patients with hepatic impairment, as both components are metabolized by the liver. Dosage adjustment may be necessary.
  • Kidney - Use with caution in patients with severe renal impairment or renal artery stenosis due to the telmisartan component. Monitor kidney function closely.
  • Lung - Use with caution in patients with asthma, COPD, or other bronchospastic conditions due to the beta-blocker component, which can worsen respiratory symptoms.
