About

Ticagrelor, marketed as TORPLAT 90, is an oral antiplatelet agent belonging to the class of P2Y12 platelet inhibitors. It is indicated for the prevention of atherothrombotic events in adult patients with acute coronary syndrome (ACS) or a history of myocardial infarction and a high risk of developing a thrombotic event. Unlike clopidogrel, ticagrelor is a direct-acting, reversibly binding P2Y12 receptor antagonist that does not require metabolic activation. This results in a faster onset of action and more consistent antiplatelet effect.

By inhibiting platelet aggregation, ticagrelor reduces the risk of cardiovascular death, myocardial infarction, and stroke in high-risk patients. Its efficacy has been demonstrated in large clinical trials, establishing it as a crucial component of dual antiplatelet therapy (DAPT) alongside aspirin for these indications.

Uses

  • Prevention of atherothrombotic events in acute coronary syndrome (ACS).
  • Reduction of cardiovascular death, myocardial infarction, and stroke.
  • Prevention of stent thrombosis after percutaneous coronary intervention (PCI).
  • Used in conjunction with aspirin for dual antiplatelet therapy.

Directions For Use

Take one tablet orally twice daily, with or without food. Do not stop treatment without consulting your doctor.

Benefits

  • Rapid onset of antiplatelet action.
  • More consistent antiplatelet effect than some alternatives.
  • Significantly reduces risk of major cardiovascular events.
  • Effective in patients with acute coronary syndrome.
  • Does not require metabolic activation.
  • Reversible binding to P2Y12 receptor.

Side Effects

  • Bleeding (e.g., bruising, nosebleeds)
  • Dyspnea (shortness of breath)
  • Headache
  • Nausea
  • Diarrhea
  • Dizziness
  • Bradycardia
  • Hypotension
  • Rash

  • Syncope

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can increase the risk of bleeding, which is a known side effect of ticagrelor.
  • Pregnancy - Not recommended during pregnancy due to potential fetal harm and increased bleeding risk. Consult a doctor.
  • Breastfeeding - Avoid during breastfeeding as ticagrelor may pass into breast milk and pose a risk to the infant.
  • Liver - Use with caution in patients with moderate to severe hepatic impairment; dosage adjustment may be necessary.
  • Kidney - No dosage adjustment is generally required for renal impairment, but monitor closely in severe cases.
  • Lung - Use with caution in patients with asthma or COPD, as dyspnea is a common side effect.
