About

Nitrofurantoin is a synthetic nitrofuran antibacterial agent specifically indicated for the treatment and prophylaxis of uncomplicated urinary tract infections (UTIs) caused by susceptible bacterial strains. Its unique mechanism of action involves its reduction by bacterial flavoproteins to highly reactive intermediates. These intermediates then inactivate or damage multiple bacterial macromolecules, including ribosomal proteins, DNA, RNA, and enzymes involved in metabolic processes. This multi-target action is crucial as it significantly minimizes the development of bacterial resistance, a growing concern with many other antibiotics. Nitrofurantoin is rapidly absorbed and excreted primarily in the urine, where it achieves high therapeutic concentrations, making it highly effective against pathogens localized within the urinary tract while having minimal systemic antibacterial effects. It is particularly effective against E. coli and Enterococcus species.

Uses

  • Treatment of acute uncomplicated UTIs.
  • Prophylaxis of recurrent UTIs.
  • Management of cystitis.
  • Prevention of UTIs after certain procedures.

Directions For Use

Take orally with food or milk to enhance absorption and reduce gastrointestinal upset, as directed by your doctor.

Benefits

  • Highly effective against common UTI pathogens.
  • Low resistance development due to multi-target action.
  • Achieves high concentrations in urine.
  • Suitable for long-term prophylaxis.
  • Minimal systemic side effects.
  • Generally well-tolerated.

Side Effects

  • Nausea
  • Vomiting
  • Diarrhea
  • Abdominal pain
  • Headache
  • Dizziness
  • Darkening of urine (harmless)
  • Rash
  • Pulmonary reactions (acute or chronic, rare)
  • Hepatotoxicity (rare)
  • Peripheral neuropathy (rare, prolonged use)
  • Hemolytic anemia (in G6PD deficiency)

Safety Measures

  • Alcohol - No specific interaction with alcohol, but it's generally advisable to avoid alcohol during antibiotic treatment to support recovery.
  • Pregnancy - Generally considered safe in the first and second trimesters for uncomplicated UTIs. Avoid near term (38-42 weeks) due to risk of hemolytic anemia in the newborn.
  • Breastfeeding - Excreted in very small amounts into breast milk. Generally considered compatible with breastfeeding, but avoid if infant has G6PD deficiency.
  • Liver - Use with caution in patients with pre-existing liver disease; rare cases of hepatotoxicity have been reported.
  • Kidney - Contraindicated in patients with severe renal impairment (CrCl < 60 mL/min) due to reduced efficacy and increased risk of toxicity.
  • Lung - Use with caution in patients with pre-existing lung conditions; rare but serious pulmonary reactions (fibrosis, pneumonitis) can occur.
