About

Abiraterone acetate is an oral androgen biosynthesis inhibitor that specifically targets cytochrome P450 17α-hydroxylase/C17,20-lyase (CYP17), an enzyme expressed in testicular, adrenal, and prostatic tumor tissues. By inhibiting CYP17, abiraterone significantly reduces the production of androgens, including testosterone, which are crucial for the growth of prostate cancer cells. This mechanism of action makes it effective in treating metastatic castration-resistant prostate cancer (mCRPC).

It is typically administered in combination with prednisone or prednisolone to mitigate mineralocorticoid excess, a common side effect resulting from increased adrenocorticotropic hormone (ACTH) levels due to CYP17 inhibition. Clinical trials have demonstrated that abiraterone prolongs overall survival and progression-free survival in patients with mCRPC, both before and after chemotherapy.

Uses

  • Metastatic castration-resistant prostate cancer (mCRPC)
  • High-risk metastatic hormone-sensitive prostate cancer (mHSPC)
  • Prostate cancer progression despite androgen deprivation therapy
  • Treatment in combination with prednisone/prednisolone

Directions For Use

Take orally on an empty stomach, at least one hour before or two hours after food. Swallow the tablet whole with water; do not crush or chew.

Benefits

  • Prolongs overall survival in advanced prostate cancer.
  • Delays disease progression in metastatic settings.
  • Reduces androgen production, starving cancer cells.
  • Effective in both pre- and post-chemotherapy settings.
  • Improves quality of life for patients.
  • Oral administration offers convenience.

Side Effects

  • Fatigue

  • Fluid retention (edema)
  • Urinary tract infection
  • Liver enzyme elevations
  • Adrenal insufficiency
  • Cardiac disorders (e.g., arrhythmia)

Safety Measures

  • Alcohol - Avoid or limit alcohol consumption as it may exacerbate liver side effects and potentially interact with the medication's metabolism.
  • Pregnancy - Abiraterone is contraindicated in pregnant women due to potential harm to the fetus. Women of childbearing potential should avoid contact with crushed or broken tablets.
  • Breastfeeding - It is unknown if abiraterone is excreted in human milk. Due to the potential for serious adverse reactions in breastfed infants, breastfeeding is not recommended during treatment.
  • Liver - Use with caution in patients with pre-existing liver impairment. Regular monitoring of liver function tests is essential, and dose adjustments or discontinuation may be necessary.
  • Kidney - Generally, no dose adjustment is required for mild to moderate renal impairment. Use with caution in severe renal impairment, and monitor for adverse effects.
  • Lung - While not a primary concern, patients should report any new or worsening respiratory symptoms, though severe lung-related side effects are rare.
