About

Celbira (capecitabine) is an oral fluoropyrimidine carbamate that acts as a prodrug of 5-fluorouracil (5-FU), a cytotoxic agent widely used in cancer chemotherapy. Capecitabine is selectively activated in tumor cells by a three-step enzymatic pathway, with the final step catalyzed by thymidine phosphorylase, an enzyme often found in higher concentrations in many tumors than in normal tissues. Once converted to 5-FU, it interferes with DNA synthesis and RNA function, leading to cell cycle arrest and apoptosis in rapidly dividing cancer cells.

This targeted activation aims to maximize efficacy against cancer cells while minimizing systemic toxicity to healthy tissues, making it a valuable option for various solid tumors. Its oral administration offers convenience and improved quality of life for patients compared to intravenous chemotherapy, making it a cornerstone in the management of several common malignancies.

Uses

  • Treatment of metastatic colorectal cancer.
  • Adjuvant treatment of colon cancer.
  • Treatment of metastatic breast cancer.
  • Treatment of advanced gastric cancer.

Directions For Use

Administer in two divided doses, usually morning and evening, as prescribed.

  • Selective activation in tumor cells may reduce systemic toxicity.
  • Effective against various solid tumors.
  • Can be used as monotherapy or in combination regimens.
  • Improves progression-free and overall survival in some cancers.
  • Well-established efficacy profile in oncology.

Side Effects

  • Diarrhea.

Safety Measures

  • Alcohol - Avoid or limit alcohol consumption during treatment as it may exacerbate gastrointestinal side effects and could potentially interact with the medication.
  • Pregnancy - Contraindicated during pregnancy due to potential fetal harm. Women of childbearing potential must use effective contraception during treatment and for 6 months after the last dose.
  • Breastfeeding - Not recommended during breastfeeding. Capecitabine and its metabolites are likely excreted in human milk and could cause serious adverse reactions in breastfed infants.
  • Liver - Use with caution in patients with mild to moderate hepatic impairment. Close monitoring of liver function tests is required, and dose adjustments may be necessary.
  • Kidney - Requires dose reduction in patients with moderate to severe renal impairment (creatinine clearance <60 mL/min) due to increased systemic exposure to metabolites.
  • Lung - Rare but serious side effects like interstitial pneumonitis have been reported. Patients should be monitored for new or worsening respiratory symptoms such as cough or dyspnea.
