About

Daratumumab, marketed as Darzalex, is a human CD38-directed monoclonal antibody indicated for the treatment of multiple myeloma. CD38 is a transmembrane glycoprotein that is highly expressed on the surface of multiple myeloma cells, as well as on other hematopoietic and lymphoid cells. Daratumumab binds to CD38 on myeloma cells and induces cell death through multiple mechanisms, including complement-dependent cytotoxicity (CDC), antibody-dependent cell-mediated cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), and apoptosis.

This targeted therapy also modulates the activity of certain immune suppressor cells, such as regulatory T cells, myeloid-derived suppressor cells, and B cells, which also express CD38. By depleting these cells, Daratumumab may enhance anti-tumor immune responses. It is often used in combination with other agents like lenalidomide, bortezomib, and dexamethasone, or as a monotherapy in patients who have received prior therapies. Its introduction has significantly improved outcomes for patients with relapsed or refractory multiple myeloma, offering a novel approach to disease management.
