About

Telmisartan is an angiotensin II receptor blocker (ARB) indicated for the treatment of essential hypertension and for cardiovascular risk reduction in patients unable to take ACE inhibitors. It works by selectively blocking the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This blockade prevents the potent vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to systemic vasodilation, reduced blood pressure, and decreased fluid retention. Telmisartan has a long duration of action, allowing for convenient once-daily dosing, and is known for its high lipophilicity and long plasma half-life, contributing to its sustained antihypertensive effect. Its proven efficacy in blood pressure control and its potential renoprotective effects make it a valuable agent in comprehensive cardiovascular management, particularly for patients at high risk of cardiovascular events.

Uses

  • Treatment of essential hypertension.
  • Reduction of cardiovascular morbidity and mortality.
  • Prevention of cardiovascular events in high-risk patients.
  • Management of diabetic nephropathy (off-label in some regions).

Directions For Use

Benefits

  • Effective blood pressure reduction.
  • Long-lasting effect (once-daily dosing).
  • Reduces risk of cardiovascular events.
  • Well-tolerated in most patients.
  • May be renoprotective.
  • Lower incidence of cough compared to ACE inhibitors.

Side Effects

  • Hypotension
  • Fatigue
  • Headache
  • Back pain
  • Upper respiratory tract infection
  • Diarrhea
  • Nausea
  • Hyperkalemia
  • Angioedema (rare)

Safety Measures

  • Alcohol - Alcohol can increase the blood pressure-lowering effect of telmisartan, potentially leading to dizziness or fainting. Use with caution.
  • Pregnancy - Contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury and death. Discontinue immediately if pregnancy is detected.
  • Breastfeeding - Not recommended during breastfeeding as telmisartan may be excreted in human milk and could harm the infant.
  • Liver - Use with caution in patients with severe hepatic impairment or biliary obstructive disorders, as telmisartan is primarily eliminated via bile.
  • Kidney - Use with caution in patients with renal artery stenosis or severe renal impairment, as it may worsen kidney function. Monitor renal function.
  • Lung - No specific contraindications related to lung conditions. It is generally well-tolerated in patients with respiratory issues.
