Gliotem (Temozolomide) is an oral alkylating agent primarily used in the treatment of glioblastoma multiforme and anaplastic astrocytoma, aggressive forms of brain cancer. As a prodrug, temozolomide is rapidly converted at physiological pH to its active metabolite, MTIC (5-(3-methyl-1-triazen-1-yl)imidazole-4-carboxamide). This active metabolite then methylates DNA at specific sites, leading to DNA damage and ultimately triggering apoptosis (programmed cell death) in tumor cells. Its ability to cross the blood-brain barrier makes it particularly effective for central nervous system malignancies. Temozolomide is often used in combination with radiation therapy, especially in newly diagnosed glioblastoma, to enhance therapeutic outcomes and improve patient survival.
