About
Gluvilda DM 5/500 is a combination medication used in the management of type 2 diabetes mellitus. It contains Vildagliptin (5mg), a dipeptidyl peptidase-4 (DPP-4) inhibitor, and Metformin (500mg), a biguanide. Vildagliptin works by inhibiting the DPP-4 enzyme, which increases the levels of incretin hormones (GLP-1 and GIP). These hormones enhance glucose-dependent insulin secretion from pancreatic beta cells and suppress glucagon secretion from alpha cells, thereby lowering blood glucose. Metformin primarily reduces hepatic glucose production and improves insulin sensitivity in peripheral tissues, leading to decreased glucose absorption from the gastrointestinal tract. This dual mechanism of action provides comprehensive glycemic control, helping patients achieve target blood sugar levels when diet and exercise alone are insufficient.
This synergistic combination addresses multiple pathophysiological defects in type 2 diabetes, offering improved efficacy compared to monotherapy. It is particularly beneficial for patients who require more intensive glycemic control or those who have not achieved adequate control with Metformin alone. The medication helps to reduce both fasting and post-prandial glucose levels, contributing to better long-term outcomes and a reduced risk of diabetes-related complications.
Uses
- Management of type 2 diabetes mellitus.
- Improvement of glycemic control in adults.
- Used as an adjunct to diet and exercise.
- For patients inadequately controlled on Metformin alone.
Directions For Use
Take this medicine as directed by your doctor, usually with meals to reduce gastrointestinal side effects. Swallow the tablet whole with water.
Benefits
- Effective blood sugar control.
- Reduces both fasting and post-meal glucose.
- Improved insulin sensitivity.
- Lower risk of hypoglycemia compared to some other agents.
- May support weight management.
- Convenient combination therapy.
Side Effects
- Nausea
- Diarrhea
- Abdominal pain
- Vomiting
- Headache
- Upper respiratory tract infection
- Sore throat
- Nasal congestion
- Joint pain
- Lactic acidosis (rare but serious)
- Vitamin B12 deficiency (long-term Metformin use)
- Pancreatitis (rare)
Safety Measures
- Alcohol - Avoid excessive alcohol consumption as it can increase the risk of lactic acidosis, a serious side effect, especially when combined with Metformin.
- Pregnancy - Generally not recommended during pregnancy. Consult your doctor, as benefits must outweigh risks, and insulin may be preferred.
- Breastfeeding - Metformin passes into breast milk. Consult your doctor to weigh the potential risks to the infant against the benefits to the mother.
- Liver - Use with caution in patients with liver impairment, as it may increase the risk of lactic acidosis due to reduced Metformin clearance.
- Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²). Dose adjustment is required for moderate impairment due to Metformin accumulation risk.
- Lung - No specific contraindications related to lung conditions, but severe respiratory failure can increase lactic acidosis risk.
