About

Linagliptin/Metformin is a fixed-dose combination medication indicated for the comprehensive management of type 2 diabetes mellitus in adults. Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that works by increasing the levels of active incretin hormones, such as GLP-1 and GIP. These hormones are crucial for glucose homeostasis, as they enhance glucose-dependent insulin release from pancreatic beta cells and suppress glucagon secretion from alpha cells, thereby lowering blood glucose concentrations.

Metformin, a biguanide, complements this action by primarily reducing hepatic glucose production, mainly through inhibiting gluconeogenesis and glycogenolysis in the liver. It also improves insulin sensitivity in peripheral tissues, leading to increased glucose uptake and utilization by muscle cells, and decreases intestinal absorption of glucose. The synergistic action of linagliptin and metformin addresses multiple pathophysiological defects of type 2 diabetes, providing robust glycemic control, reducing HbA1c levels, and offering a low risk of hypoglycemia when not combined with insulin secretagogues.

Uses

  • Treatment of type 2 diabetes mellitus.
  • To improve glycemic control in adults.
  • Used as an adjunct to diet and exercise.
  • When both linagliptin and metformin are appropriate.

Directions For Use

Take this medication orally, usually twice daily with meals, to minimize gastrointestinal side effects, or as advised by your healthcare provider. Swallow the tablet whole.

Benefits

  • Provides comprehensive glycemic control.
  • Effectively lowers HbA1c levels.
  • Reduces hepatic glucose production.
  • Enhances insulin sensitivity.
  • Low risk of hypoglycemia (as monotherapy).
  • Contributes to modest weight stability or loss.

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal pain
  • Vomiting
  • Headache
  • Nasopharyngitis
  • Hypoglycemia (when combined with sulfonylureas/insulin)
  • Lactic acidosis (rare but serious, due to metformin)
  • Pancreatitis (rare, due to linagliptin)
  • Joint pain (arthralgia)
  • Skin reactions (e.g., rash)
  • Vitamin B12 deficiency (long-term metformin use)

Safety Measures

  • Alcohol - Avoid excessive alcohol intake, as it can increase the risk of lactic acidosis (due to metformin) and hypoglycemia.
  • Pregnancy - Not recommended during pregnancy. Insulin is generally preferred for glycemic control in pregnant women with diabetes. Consult a doctor.
  • Breastfeeding - Avoid use during breastfeeding as both components are excreted in milk and could pose risks to the infant.
  • Liver - Use with caution in patients with liver impairment. Metformin is contraindicated in severe hepatic impairment due to increased risk of lactic acidosis.
  • Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²). Dose adjustment is required for moderate impairment.
  • Lung - Use with caution in conditions causing hypoxia (e.g., severe lung disease, acute heart failure) as these can increase the risk of lactic acidosis.
