About
Nepsita 50/500 is a combination medication containing Vildagliptin (50mg) and Metformin (500mg), indicated for the management of type 2 diabetes mellitus. Metformin, a biguanide, works by reducing hepatic glucose production, decreasing intestinal glucose absorption, and improving insulin sensitivity in peripheral tissues. Vildagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, enhances the body's natural ability to control blood glucose by increasing the levels of active incretin hormones (GLP-1 and GIP).
These incretins stimulate insulin release from pancreatic beta cells and suppress glucagon secretion from alpha cells in a glucose-dependent manner, leading to improved glycemic control. The synergistic action of Vildagliptin and Metformin addresses multiple pathophysiological defects of type 2 diabetes, providing comprehensive blood glucose lowering effects and helping to achieve target HbA1c levels when monotherapy is insufficient.
Uses
- Management of type 2 diabetes mellitus.
- As an adjunct to diet and exercise.
- When metformin monotherapy is insufficient.
- To improve glycemic control in adults.
Directions For Use
Take orally with meals to minimize gastrointestinal side effects. Follow your doctor's prescribed dosage and schedule.
Benefits
- Provides effective glycemic control.
- Reduces both fasting and post-meal blood sugar.
- Lowers HbA1c levels significantly.
- Offers a dual mechanism of action.
- Generally well-tolerated.
- May have a low risk of hypoglycemia.
Side Effects
- Nausea
- Vomiting
- Diarrhea
- Abdominal pain
- Metallic taste
- Headache
- Dizziness
- Nasopharyngitis
- Tremor

- Hypoglycemia (rare)
- Lactic acidosis (rare, serious)
Safety Measures
- Alcohol - Avoid excessive alcohol intake due to an increased risk of lactic acidosis, especially with metformin.
- Pregnancy - Generally not recommended during pregnancy unless the potential benefits outweigh the risks. Consult your doctor.
- Breastfeeding - Metformin passes into breast milk; vildagliptin data is limited. Consult a healthcare professional before use.
- Liver - Use with caution; metformin is contraindicated in severe hepatic impairment. Vildagliptin is not recommended in moderate to severe impairment.
- Kidney - Dose adjustment is required; contraindicated in severe renal impairment due to metformin accumulation and increased risk of lactic acidosis.
- Lung - No specific contraindication, but caution is advised in severe respiratory conditions that may increase the risk of acidosis.
