About

Palbociclib is an oral cyclin-dependent kinase (CDK) 4/6 inhibitor indicated for the treatment of hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced or metastatic breast cancer. It is typically used in combination with an aromatase inhibitor as initial endocrine-based therapy in postmenopausal women or men, or with fulvestrant in patients with disease progression following endocrine therapy. The drug works by selectively inhibiting CDK4 and CDK6, key enzymes that regulate cell cycle progression. By blocking these kinases, palbociclib prevents cancer cells from advancing from the G1 phase to the S phase of the cell cycle, thereby inhibiting their proliferation.

This targeted therapy plays a crucial role in extending progression-free survival in eligible patients with advanced breast cancer. Its mechanism of action specifically targets the dysregulated cell cycle machinery often found in HR-positive breast cancers, offering a more precise approach compared to traditional chemotherapy. Regular monitoring of blood counts is essential during treatment due to the common occurrence of neutropenia.

Uses

  • Treatment of HR-positive, HER2-negative advanced or metastatic breast cancer.
  • Used in combination with an aromatase inhibitor as initial endocrine-based therapy.
  • Used with fulvestrant in patients whose disease has progressed after endocrine therapy.
  • Helps to slow the progression of certain types of breast cancer.

Directions For Use

Take this medication orally as directed by your doctor, usually once daily for 21 consecutive days, followed by 7 days off, to complete a 28-day cycle. It should be taken with food.

Safety Measures

  • Alcohol - It is generally advisable to limit or avoid alcohol consumption while taking palbociclib, as it may exacerbate certain side effects like nausea or fatigue, and potential interactions are not fully established.
  • Pregnancy - Palbociclib is contraindicated during pregnancy as it can cause fetal harm. Women of childbearing potential must use effective contraception during treatment and for at least 3 weeks after the last dose.
  • Breastfeeding - Breastfeeding is not recommended during treatment with palbociclib and for at least 3 weeks after the last dose, due to the potential for serious adverse reactions in breastfed infants.
  • Liver - Use with caution in patients with liver conditions. Dose adjustments may be necessary for those with severe hepatic impairment, as palbociclib is primarily metabolized in the liver.
  • Kidney - Use with caution in patients with kidney conditions. Dose adjustments may be required for individuals with severe renal impairment, as renal excretion plays a role in drug elimination.
  • Lung - Patients should be monitored for new or worsening respiratory symptoms, as palbociclib can rarely cause interstitial lung disease (ILD) or pneumonitis. Seek immediate medical attention if symptoms like shortness of breath or cough develop.
