About
Sacubitril/Valsartan is an innovative combination medication comprising a neprilysin inhibitor (sacubitril) and an angiotensin receptor blocker (valsartan). It is primarily indicated for the treatment of chronic heart failure with reduced ejection fraction (HFrEF) in adult patients, and also in pediatric patients aged one year or older with symptomatic chronic heart failure. Sacubitril works by inhibiting neprilysin, an enzyme responsible for degrading natriuretic peptides, leading to increased levels of these beneficial peptides that promote vasodilation and natriuresis. Valsartan, an angiotensin II receptor blocker, counteracts the detrimental effects of the renin-angiotensin-aldosterone system. This dual, synergistic action helps to significantly reduce cardiovascular mortality and the frequency of heart failure hospitalizations, thereby improving the overall prognosis and quality of life for patients with HFrEF.
Uses
- Treatment of chronic heart failure with reduced ejection fraction.
- Reduction of cardiovascular mortality in HFrEF patients.
- Decrease in heart failure hospitalizations.
- Management of symptomatic chronic heart failure.
Directions For Use
Take orally twice daily, usually in the morning and evening, as directed by your cardiologist.
Benefits
- Significantly reduces cardiovascular mortality.
- Lowers risk of heart failure hospitalizations.
- Improves symptoms and quality of life in HFrEF.
- Provides dual mechanism of action.
- Superior to ACE inhibitors in certain heart failure outcomes.
- Offers comprehensive heart failure management.
Side Effects
- Hypotension
- Hyperkalemia
- Cough
- Dizziness
- Renal impairment
- Fatigue
- Nausea
- Diarrhea
- Angioedema (rare but serious)
- Anemia
- Syncope
- Orthostatic hypotension
Safety Measures
- Alcohol - Avoid excessive alcohol as it can exacerbate hypotension and dizziness, especially at treatment initiation.
- Pregnancy - Contraindicated during pregnancy due to significant risk of fetal injury or death; discontinue immediately if pregnancy is detected.
- Breastfeeding - Not recommended; it is unknown if components are excreted in human milk, but potential for serious adverse effects on the infant exists.
- Liver - Use with caution in patients with moderate hepatic impairment; not recommended in severe hepatic impairment.
- Kidney - Monitor renal function closely, especially in patients with pre-existing renal impairment; dose adjustment may be needed.
