About

TIBAN M20/500 is a combination medication used in the management of type 2 diabetes mellitus. It combines Teneligliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and Metformin, a biguanide. Teneligliptin works by increasing the levels of incretin hormones, which stimulate insulin release and suppress glucagon secretion in a glucose-dependent manner, thereby lowering blood glucose. Metformin primarily reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. This dual mechanism of action provides comprehensive glycemic control.

This combination therapy is typically prescribed when diet and exercise alone, or monotherapy with either Teneligliptin or Metformin, do not achieve adequate glycemic targets. It helps to lower both fasting and post-meal blood sugar levels, contributing to better long-term management of diabetes and reduction in associated complications, making it a valuable tool in diabetes care.

Uses

  • Management of type 2 diabetes mellitus.
  • Improvement of glycemic control in adults.
  • Used as an adjunct to diet and exercise.
  • When monotherapy is insufficient.

Directions For Use

Take this medication orally as directed by your physician, usually with meals to minimize gastrointestinal side effects.

Swallow the tablet whole with water; do not crush or chew.

Benefits

  • Effective in lowering blood glucose levels.
  • Improves both fasting and post-prandial glucose.
  • Dual mechanism of action for comprehensive control.
  • May help reduce HbA1c levels significantly.
  • Generally well-tolerated with proper use.
  • Convenient fixed-dose combination.

Side Effects

  • Nausea
  • Diarrhea
  • Vomiting
  • Abdominal pain
  • Headache
  • Dizziness
  • Hypoglycemia (especially with other antidiabetics)
  • Metallic taste in mouth
  • Upper respiratory tract infection
  • Nasopharyngitis
  • Lactic acidosis (rare but serious)
  • Vitamin B12 deficiency (long-term use)

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can increase the risk of lactic acidosis, a rare but serious side effect, especially with metformin.
  • Pregnancy - Generally not recommended during pregnancy unless clearly necessary and benefits outweigh risks. Consult a doctor.
  • Breastfeeding - Metformin is excreted in breast milk; Teneligliptin data is limited. Use with caution and under medical supervision.
  • Liver - Use with caution in patients with liver impairment; severe liver disease is a contraindication for metformin due to increased lactic acidosis risk.
  • Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²) due to increased risk of metformin accumulation and lactic acidosis.
  • Lung - No specific contraindications related to lung conditions, but severe respiratory insufficiency can increase lactic acidosis risk.
