About
This medication combines three distinct antihypertensive agents: Telmisartan, an angiotensin II receptor blocker (ARB); Amlodipine, a dihydropyridine calcium channel blocker (CCB); and Hydrochlorothiazide, a thiazide diuretic. Telmisartan works by blocking the effects of angiotensin II, leading to vasodilation and reduced blood pressure. Amlodipine relaxes the smooth muscles in blood vessel walls, further promoting vasodilation and decreasing peripheral resistance. Hydrochlorothiazide increases the excretion of sodium and water by the kidneys, reducing blood volume and consequently blood pressure.
This triple combination therapy is designed for patients whose blood pressure is not adequately controlled by monotherapy or dual therapy. By targeting different pathways involved in blood pressure regulation, it provides a comprehensive and potent approach to achieve optimal blood pressure control, thereby reducing the risk of cardiovascular events such as stroke and heart attack.
Uses
- Treatment of essential hypertension.
- Management of uncontrolled high blood pressure.
- Reduction of cardiovascular risk in hypertensive patients.
- When monotherapy or dual therapy is insufficient.

- Targets multiple pathways of hypertension.
- Reduces risk of stroke and heart attack.
- Convenient once-daily dosing.
- Effective for resistant hypertension.
- Improved patient adherence due to combination.
Side Effects
- Dizziness

- Diarrhea
- Hypotension
- Electrolyte imbalances (e.g., hypokalemia)

Safety Measures
- Alcohol - Avoid alcohol as it can enhance the blood pressure-lowering effect and increase the risk of dizziness.
- Pregnancy - Contraindicated during the second and third trimesters due to risk of fetal harm from telmisartan.
- Breastfeeding - Not recommended during breastfeeding as components may pass into breast milk and affect the infant.
- Liver - Use with caution in patients with severe liver impairment; monitor liver function tests.
- Kidney - Contraindicated in anuria; use with caution and adjust dose in patients with severe kidney impairment; monitor renal function and electrolytes.
- Lung - Generally safe, but hydrochlorothiazide can rarely exacerbate acute gout or systemic lupus erythematosus, which might have pulmonary manifestations.
