About

Cilnidipine is a unique dihydropyridine calcium channel blocker (CCB) primarily indicated for the treatment of essential hypertension. Its distinctive mechanism of action involves blocking both L-type and N-type voltage-dependent calcium channels. The blockade of L-type channels in vascular smooth muscle cells leads to potent peripheral vasodilation, effectively reducing systemic vascular resistance and consequently lowering blood pressure. Crucially, its N-type calcium channel blocking activity helps to modulate sympathetic nervous system overactivity, which is often implicated in the pathophysiology of hypertension and associated organ damage. This dual L-type and N-type channel blockade provides comprehensive blood pressure control and may offer additional benefits such as renoprotection and reduced incidence of reflex tachycardia and ankle edema, distinguishing it from conventional L-type CCBs.

Uses

  • Essential hypertension.
  • Hypertension with associated renal disease.
  • Hypertension with sympathetic overactivity.
  • Management of isolated systolic hypertension.

Directions For Use

Take orally once daily, preferably in the morning after food, or as directed by your physician.
Swallow the tablet whole with water.

Benefits

  • Effective in lowering blood pressure.
  • Provides dual L-type and N-type calcium channel blockade.
  • May offer renoprotective benefits.
  • Helps to suppress sympathetic nervous system activity.
  • Generally well-tolerated with a low incidence of ankle edema.
  • Suitable for long-term management of hypertension.

Side Effects

  • Headache
  • Dizziness
  • Palpitations
  • Flushing
  • Ankle edema (less common than other CCBs)
  • Nausea
  • Fatigue
  • Hypotension
  • Rash
  • Gingival hyperplasia (rare)
  • Abdominal discomfort
  • Increased heart rate (reflex tachycardia, less common)

Safety Measures

  • Alcohol - Alcohol can enhance the blood pressure-lowering effect of cilnidipine, potentially leading to excessive hypotension and dizziness. Avoid or limit alcohol consumption.
  • Pregnancy - Cilnidipine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Consult a doctor.
  • Breastfeeding - It is unknown if cilnidipine is excreted in human milk. Caution should be exercised when administered to a breastfeeding woman.
  • Liver - Use with caution in patients with hepatic impairment, as cilnidipine is metabolized in the liver. Dose adjustment may be necessary.
  • Kidney - No dose adjustment is typically required in patients with renal impairment. However, monitor blood pressure closely.
  • Lung - Cilnidipine does not have direct significant effects on lung function. No specific precautions are typically needed for patients with lung conditions.
