About

This medication combines three active agents: Dapagliflozin, a sodium-glucose co-transporter 2 (SGLT2) inhibitor; Glibenclamide, a sulfonylurea; and Metformin, a biguanide. Dapagliflozin works by increasing glucose excretion in the urine, thereby lowering blood glucose levels independently of insulin. Glibenclamide stimulates insulin release from pancreatic beta cells, primarily when blood glucose levels are elevated. Metformin reduces hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity in peripheral tissues. This triple combination provides a comprehensive approach to glycemic control in adults with type 2 diabetes mellitus, targeting multiple pathophysiological pathways involved in the disease. It is typically used when diet, exercise, and dual therapy do not provide adequate glycemic control.

Uses

  • Management of type 2 diabetes mellitus.
  • Improvement of glycemic control.
  • Reduction of HbA1c levels.
  • Used as an adjunct to diet and exercise.

Directions For Use

Take this medication orally, usually once or twice daily, as prescribed by your doctor. It is generally recommended to take it with meals to minimize gastrointestinal side effects.

Benefits

  • Comprehensive blood glucose reduction.
  • Improved insulin sensitivity.
  • Reduced hepatic glucose production.
  • Increased urinary glucose excretion.
  • Potential for weight reduction (due to Dapagliflozin).
  • Lowered risk of certain cardiovascular events (due to Dapagliflozin).

Side Effects

  • Hypoglycemia (low blood sugar)
  • Gastrointestinal disturbances (nausea, diarrhea, abdominal pain)
  • Urinary tract infections
  • Genital yeast infections
  • Dehydration
  • Dizziness
  • Headache
  • Lactic acidosis (rare, but serious)
  • Pancreatitis (rare)
  • Joint pain
  • Increased urination
  • Taste disturbance

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can increase the risk of lactic acidosis, especially with Metformin, and may also increase the risk of hypoglycemia.
  • Pregnancy - Generally not recommended during pregnancy. Consult your doctor, as alternative treatments may be necessary to manage diabetes safely during this period.
  • Breastfeeding - Not recommended during breastfeeding as components may pass into breast milk and potentially harm the infant. Discuss with your healthcare provider.
  • Liver - Use with caution in patients with liver impairment, as it may increase the risk of lactic acidosis (Metformin) and affect drug metabolism. Regular monitoring is advised.
  • Kidney - Contraindicated in severe renal impairment due to the risk of lactic acidosis (Metformin) and reduced efficacy/increased risk of adverse effects (Dapagliflozin). Dose adjustment is required for moderate impairment.
  • Lung - Use with caution in patients with severe lung conditions, as these may predispose to hypoxia and increase the risk of lactic acidosis.
