About

Eltrombopag is an orally active, small-molecule thrombopoietin receptor agonist that stimulates the proliferation and differentiation of megakaryocytes, thereby increasing platelet production. It is indicated for the treatment of thrombocytopenia in various clinical settings where low platelet counts pose a significant risk of bleeding. Its mechanism involves binding to the thrombopoietin receptor (TPO-R) on hematopoietic stem cells and megakaryocyte progenitor cells, mimicking the action of endogenous thrombopoietin but acting at a different binding site. This leads to a dose-dependent increase in platelet counts, reducing the need for platelet transfusions and mitigating bleeding events.

Eltrombopag is primarily used in chronic immune thrombocytopenia (ITP) patients who have had an insufficient response to corticosteroids, immunoglobulins, or splenectomy. It is also approved for the treatment of thrombocytopenia in patients with chronic hepatitis C virus (HCV) infection to allow the initiation and maintenance of interferon-based therapy, and for severe aplastic anemia (SAA) in patients who have had an insufficient response to immunosuppressive therapy. Careful monitoring of liver function and platelet counts is essential during treatment.

Uses

  • Treatment of chronic immune thrombocytopenia (ITP).
  • Management of thrombocytopenia in chronic hepatitis C virus (HCV) infection.
  • Treatment of severe aplastic anemia (SAA).
  • Prevention of bleeding episodes due to low platelet counts.

Directions For Use

Take orally once daily on an empty stomach, at least 2 hours before or 4 hours after calcium-containing products, antacids, or dairy. Follow your doctor's prescribed dosage and duration.

Benefits

  • Significantly increases platelet counts.
  • Reduces the risk of bleeding episodes.
  • Decreases the need for platelet transfusions.
  • Improves quality of life for patients with thrombocytopenia.
  • Allows for continuation of antiviral therapy in HCV patients.
  • Offers an effective option for refractory severe aplastic anemia.

Side Effects

  • Nausea
  • Diarrhea
  • Upper respiratory tract infection
  • Fatigue
  • Fever
  • Headache
  • Cough
  • Abdominal pain
  • Increased liver enzymes
  • Muscle pain
  • Thromboembolic events (e.g., DVT, PE)
  • Cataracts

Safety Measures

  • Alcohol - Avoid or limit alcohol consumption as it may exacerbate liver enzyme elevations, a known side effect of eltrombopag.
  • Pregnancy - Eltrombopag is generally not recommended during pregnancy unless the potential benefit outweighs the risk, as animal studies show reproductive toxicity.
  • Breastfeeding - It is unknown if eltrombopag is excreted in human milk; therefore, breastfeeding is generally not recommended due to potential serious adverse effects in the infant.
  • Liver - Use with caution in patients with pre-existing liver impairment; close monitoring of liver function tests is crucial, and dose adjustments may be necessary.
  • Kidney - No specific dose adjustment is typically required for patients with renal impairment, but caution is advised, and monitoring is recommended.
  • Lung - No specific contraindications or dose adjustments related to lung conditions are generally noted, but overall patient health should be considered.
