About
GALVUS MET 50/500 is a combination oral antidiabetic medication containing vildagliptin (50 mg) and metformin hydrochloride (500 mg), indicated for the treatment of type 2 diabetes mellitus. Vildagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that enhances the body's own ability to lower high blood glucose levels by increasing the levels of active incretin hormones, GLP-1 and GIP. This leads to glucose-dependent increases in insulin secretion from pancreatic beta cells and decreases in glucagon secretion from pancreatic alpha cells. Metformin, a biguanide, primarily reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. The synergistic action of these two agents provides comprehensive glycemic control, addressing multiple pathophysiological defects in type 2 diabetes, and is used as an adjunct to diet and exercise.
Uses
- Treatment of type 2 diabetes mellitus.
- Improvement of glycemic control.
- Used as an adjunct to diet and exercise.
- Monotherapy or in combination with other antidiabetic agents.
Directions For Use
Take orally, usually twice daily with meals, as directed by your physician. Swallow tablets whole with water.
Benefits
- Dual mechanism of action for comprehensive glucose control.
- Significantly reduces HbA1c levels.
- Low risk of hypoglycemia when used alone.
- May lead to modest weight loss or be weight neutral.
- Improves both fasting and post-meal glucose levels.
- Convenient fixed-dose combination.
Side Effects
- Nausea, vomiting, diarrhea.
- Abdominal pain or discomfort.
- Headache, dizziness.
- Nasopharyngitis.
- Tremor.
- Fatigue.
- Hypoglycemia (especially with other antidiabetic agents).
- Lactic acidosis (rare, serious, associated with metformin).
- Joint pain (arthralgia).
- Skin reactions (e.g., rash).
- Pancreatitis (rare).
- Decreased vitamin B12 levels with long-term use.
Safety Measures
- Alcohol - Avoid excessive alcohol intake due to an increased risk of lactic acidosis, a rare but serious side effect of metformin.
- Pregnancy - Generally not recommended during pregnancy; consult your doctor. Insulin is often preferred for glycemic control in pregnant women with diabetes.
- Breastfeeding - Not recommended during breastfeeding as metformin is excreted into breast milk and vildagliptin's excretion is unknown.
- Liver - Contraindicated in severe hepatic impairment due to the metformin component; vildagliptin is also not recommended in patients with elevated liver enzymes.
- Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²) due to the risk of metformin accumulation and lactic acidosis.
- Lung - Use with caution in severe respiratory conditions that may predispose to hypoxia, as this can increase the risk of lactic acidosis with metformin.
