About

Losartan is an angiotensin II receptor blocker (ARB) used primarily for the treatment of hypertension, heart failure, and to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy. It works by selectively blocking the binding of angiotensin II to the AT1 receptor, which is responsible for many of the deleterious effects of angiotensin II, such as vasoconstriction, aldosterone secretion, and cellular proliferation. By blocking this receptor, losartan causes vasodilation, reduces blood pressure, and decreases the workload on the heart. It is also beneficial in protecting the kidneys in patients with type 2 diabetes and nephropathy. Unlike ACE inhibitors, ARBs do not inhibit the breakdown of bradykinin, which means they are less likely to cause the persistent dry cough often associated with ACE inhibitors.

Uses

  • Treatment of essential hypertension.
  • Reduction of stroke risk in hypertensive patients with LVH.
  • Management of diabetic nephropathy in type 2 diabetes.
  • Treatment of heart failure (as an alternative to ACE inhibitors).

Directions For Use

Take this medication orally, usually once daily, with or without food, as directed by your doctor. Swallow the tablet whole with water.

Benefits

  • Effectively lowers high blood pressure.
  • Reduces the risk of stroke in specific patient populations.
  • Protects kidney function in diabetic nephropathy.
  • Well-tolerated with a lower incidence of cough than ACE inhibitors.
  • Improves cardiovascular outcomes in heart failure.
  • Provides 24-hour blood pressure control with once-daily dosing.

Side Effects

  • Dizziness
  • Fatigue
  • Hypotension (low blood pressure)
  • Hyperkalemia (high potassium levels)
  • Back pain
  • Upper respiratory infection
  • Diarrhea
  • Nausea
  • Cough (less common than ACE inhibitors)
  • Muscle cramps
  • Headache
  • Renal dysfunction (especially in susceptible patients)

Safety Measures

  • Alcohol - Alcohol can increase the blood pressure-lowering effect of losartan, potentially leading to dizziness or fainting. Limit or avoid alcohol.
  • Pregnancy - Contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury and death. Discontinue if pregnancy is detected.
  • Breastfeeding - Not recommended during breastfeeding as it may pass into breast milk and potentially harm the infant. Consult a doctor.
  • Liver - Use with caution in patients with liver impairment, as losartan is metabolized in the liver. Lower starting doses may be required.
  • Kidney - Use with caution in patients with renal artery stenosis or severe renal impairment, as it can worsen kidney function. Monitor kidney function closely.
  • Lung - Generally safe for patients with lung conditions. Unlike ACE inhibitors, it is less likely to cause cough.
