About

Mycophenolate Mofetil (MMF) is a potent immunosuppressive agent widely employed in transplant medicine to prevent the rejection of solid organ transplants, including kidney, heart, and liver allografts. Its mechanism of action involves the selective inhibition of inosine monophosphate dehydrogenase (IMPDH), a crucial enzyme in the de novo pathway of guanosine nucleotide synthesis. This inhibition specifically targets and suppresses the proliferation of T and B lymphocytes, which are key immune cells responsible for mediating organ rejection. By selectively depleting these activated lymphocytes, MMF effectively dampens the immune system's response against the transplanted organ, thereby significantly reducing the incidence of acute rejection episodes and improving long-term graft survival. It is frequently administered in combination with other immunosuppressants, such as calcineurin inhibitors and corticosteroids, to achieve optimal immunosuppression.

Uses

  • Prevention of organ rejection in kidney transplant patients.
  • Prevention of organ rejection in heart transplant patients.

  • Breastfeeding - Mycophenolate mofetil is excreted into breast milk and can cause serious adverse effects in the infant; therefore, breastfeeding is contraindicated.
  • Liver - Use with caution in patients with severe liver impairment, as it may alter drug metabolism and increase systemic exposure; dose adjustments may be needed.
  • Kidney - Use with caution in patients with severe chronic kidney impairment, as dose adjustments may be necessary, and close monitoring is required.
  • Lung - Mycophenolate mofetil can rarely cause interstitial lung disease or pulmonary fibrosis; monitor for new or worsening respiratory symptoms.
