About

Temozolomide is an oral alkylating antineoplastic agent primarily used in the treatment of certain brain tumors. As a prodrug, it undergoes rapid chemical conversion at physiological pH to its active metabolite, 5-(3-methyl-1-triazen-1-yl)imidazole-4-carboxamide (MTIC). MTIC then exerts its cytotoxic effects by methylating DNA at various sites, particularly at the O6 and N7 positions of guanine. This methylation leads to DNA damage and subsequent repair attempts by the cell. When these repair mechanisms are overwhelmed or faulty, it results in DNA strand breaks, cell cycle arrest, and ultimately apoptosis, particularly in rapidly proliferating cancer cells. Its ability to cross the blood-brain barrier makes it particularly effective for central nervous system malignancies. It is often used in combination with radiation therapy for newly diagnosed glioblastoma multiforme and as monotherapy for refractory anaplastic astrocytoma.
