About
Zomelis Met combines Vildagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and Metformin, a biguanide, to improve glycemic control in adults with type 2 diabetes mellitus. Vildagliptin works by increasing the levels of active incretin hormones, GLP-1 and GIP, which enhance insulin secretion from pancreatic beta cells and reduce glucagon secretion from alpha cells in a glucose-dependent manner. Metformin primarily reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. This dual mechanism addresses multiple pathophysiological defects in type 2 diabetes, leading to significant reductions in HbA1c levels. The combination is particularly effective for patients who have not achieved adequate glycemic control with metformin monotherapy or who are already on both drugs separately. It helps manage blood sugar levels throughout the day, both post-meal and fasting.
Uses
- Management of type 2 diabetes mellitus in adults.
- Improvement of glycemic control when diet and exercise alone are insufficient.
- Used as an add-on therapy to other antidiabetic agents.
- Initial therapy for patients with type 2 diabetes where metformin is appropriate.
Directions For Use
Take this medication orally, usually once or twice daily with meals, as directed by your doctor. Swallow the tablet whole with water.
Benefits
- Effectively lowers blood sugar levels.
- Improves insulin sensitivity.
- Reduces hepatic glucose production.
- Low risk of hypoglycemia when used alone.
- Convenient fixed-dose combination.
- Helps achieve target HbA1c levels.
Side Effects
- Nausea
- Diarrhea
- Abdominal pain
- Headache
- Dizziness
- Fatigue
- Vomiting
- Metallic taste
- Lactic acidosis (rare, with Metformin)
- Pancreatitis (rare, with Vildagliptin)
- Joint pain
- Skin reactions
Safety Measures
- Alcohol - Avoid excessive alcohol consumption as it can increase the risk of lactic acidosis, especially with Metformin, and may cause hypoglycemia.
- Pregnancy - Generally not recommended during pregnancy. Consult a doctor; benefits must outweigh potential risks.
- Breastfeeding - Not recommended during breastfeeding as components may pass into breast milk. Seek medical advice.
- Liver - Use with caution in patients with mild to moderate hepatic impairment. Contraindicated in severe liver disease due to increased risk of lactic acidosis.
- Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²). Dose adjustment required for moderate impairment.
- Lung - Use with caution in conditions causing hypoxia, which can increase the risk of lactic acidosis.
