About
Telmisartan/Chlorthalidone, known as Telvas CT, is a fixed-dose combination medication used for the treatment of essential hypertension. Telmisartan, an angiotensin II receptor blocker (ARB), works by blocking the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to a reduction in blood pressure. Chlorthalidone is a long-acting thiazide-like diuretic that increases the excretion of sodium and water by inhibiting sodium reabsorption in the cortical diluting segment of the nephron. This combination provides a powerful synergistic effect, offering superior and sustained blood pressure control compared to monotherapy. It is particularly effective in patients who require more aggressive blood pressure lowering or who have not achieved target blood pressure with a single agent, leveraging the distinct mechanisms of both components for comprehensive management.
Uses
- Treatment of essential hypertension.
- When monotherapy is insufficient for blood pressure control.
- To achieve optimal and sustained blood pressure reduction.
- To reduce the risk of cardiovascular events in hypertensive patients.
Directions For Use
Take orally once daily, with or without food, preferably in the morning, or as directed by your doctor.
Benefits
- Provides potent and sustained blood pressure reduction.
- Combines two effective antihypertensive mechanisms.
- Offers convenient once-daily dosing.
- Reduces the risk of cardiovascular morbidity and mortality.
- Effective in patients requiring more aggressive BP lowering.
- Chlorthalidone has a long duration of action.
Side Effects
- Dizziness
- Headache
- Fatigue
- Hypotension (low blood pressure)
- Hypokalemia (low potassium)
- Hyponatremia (low sodium)
- Hyperuricemia (high uric acid)
- Muscle cramps
- Nausea
- Diarrhea
- Cough (less common than with ACE inhibitors)

Safety Measures
- Alcohol - Alcohol can potentiate the hypotensive effects; use with caution and avoid excessive intake.
- Pregnancy - Contraindicated in pregnancy due to potential fetal harm from telmisartan (renal dysfunction, oligohydramnios).
- Breastfeeding - Not recommended during breastfeeding as components may pass into breast milk and harm the infant.
- Liver - Use with caution in hepatic impairment; dose adjustment may be necessary, especially with severe impairment.
- Kidney - Contraindicated in anuria; use with caution in severe renal impairment, monitor kidney function and electrolytes.
- Lung - Generally safe for use in patients with lung conditions; however, diuretics can rarely cause acute pulmonary edema.
